APPLY TO BE A VOLUNTEER IN SUMMER 2012

AMIGOS APPLICATION (PRE-INTERVIEW)

 

You must complete an application prior to your Amigos interview.  The application has two different parts:  online and paper.  You must complete BOTH of them.  

 

1. ONLINE APPLICATION 

To start the application, click on the link: “Begin an online application.”  

The application will prompt you to provide information about yourself and your interest in Amigos.  At any time, you may save your work by clicking on the box labeled “Save my progress and resume later” at the top of the page.  

To resume work on your application click on the link “Return to your online application

Complete all of the required fields and submit your application when done.

The online application requires two items that do not have to be completed prior to the interview:

a. PHOTO:  Upload a digital photo of yourself

b. PASSPORT:  Upload a scan or photo of the first page of your passport.

Your passport must be valid through December 2012.  If you do not have a valid passport, apply IMMEDIATELY!  Some post offices have passport offices.  They may require you to make an appointment.  Minors should be accompanied by one parent.  The applicant will need proof of citizenship, a previous passport or a certified (embossed) copy of your birth certificate AND picture proof of identification, like a driver’s license. You will need your social security number.  You will need two 2” photos. You may get further instructions and even download the application form by going to http://travel.state.gov.

Do not turn in hard copies of any portion of the online application at the interview.  


2.    PAPER APPLICATION 

The hard copy of the application was provided at the orientation session.  You may access a PDF version of this part of the application if you Click on the link: “Paper Application Form

You may fill this form in as a PDF document and then print it out or fill it in by hand.

BRING THE FOLLOWING COMPLETED FORMS FROM THE PAPER APPLICATION TO YOUR INTERVIEW:

a. CONSENT & RELEASE AGREEMENT (pages 4-6)

To be initialed on pages 4 and 5 and signed on page 6 by volunteer and BOTH parents.

b. RELEASE OF HEALTH INFORMATION (page 7)

To be  signed on page 7 ONLY by volunteers who are 18 or older.

c. ACKNOWLEDGEMENT OF STANDARDS OF PERSONAL CONDUCT AND COMMUNITY BEHAVIOR  (page 9)

To be signed on page 9 by volunteer and BOTH parents.

d. LETTER OF HEALTH DISCLOSURE (page 12)

To be signed on page 12 by the volunteer and one parent.

e. CONFIDENTIAL HEALTH FORM I (pages 13-15)

Fill in all information to the best of your knowledge.  To be signed on page 15 by the volunteer and one parent.  To insure confidentiality, please submit this form in a sealed envelope marked on the outside with your name.

f. FINANCIAL ASSISTANCE APPLICATION (page 23)

Optional: fill in if you are requesting financial assistance.  To be signed on page 23 by volunteer and BOTH parents.

If the volunteer is 18 years or older, no parents’ signatures are needed on any of the forms except the Financial Assistance Application.  

EVERY FORM SUBMITTED MUST BE 100% COMPLETE INCLUDING INITIALS AND SIGNATURES OF BOTH PARENTS WHERE REQUIRED—IF A FORM IS NOT COMPLETE, DO NOT TURN IT IN.
