Amigos Application for Summer 2018

To start the application, click on the link: Begin a secure online application here.

The application will prompt you to provide information about yourself and your interest in Amigos.  At any time, you may save your work by clicking on the box labeled “Save my progress and resume later” at the top of the page. 

To resume work on your application click on the link Return to your secure online application. (Link to be provided)

Complete all of the required fields, upload all your required documents, and submit your application before Interview Weekend.  

Your passport must be valid through December 2018.  If you do not have a valid passport, apply IMMEDIATELY!  Some post offices have passport offices.  They may require you to make an appointment.  Minors should be accompanied by one parent.  The applicant will need proof of citizenship, a previous passport or a certified (embossed) copy of your birth certificate AND picture proof of identification, like a driver’s license. You will need your social security number.  You will need two 2” photos. You may get further instructions and even download the application form by going to http://travel.state.gov.

2.    PAPER APPLICATION 

The hard copy of the East Bay Chapter application was provided at the information session.  If you need another copy of the paper forms, please email Lucy Schaefer, the Recruitment Director at recruitment@amigos-eastbay.org.

BRING THE FOLLOWING COMPLETED FORMS FROM THE PAPER APPLICATION TO YOUR INTERVIEW:

a. East Bay Rules of Participation

b. East Bay Amigos Chapter Medical Consent and Release

c. Each Bay Amigos Chapter Personal Information Sheet 

b. East Bay Financial Aid Application (optional)

If the volunteer is 18 years or older, no parents’ signatures are needed on any of the forms except the Financial Assistance Application.
